Office Policies
We would like to welcome you to our practice. Thank you for allowing our healthcare team the privilege to care for you. We hope this website will be helpful in answering your questions and provide you information about our office. We welcome any suggestions that may help improve our practice and service to you.
Telephone
The clinic uses an automated phone system to handle the high volume of calls received each day. Please give the receptionist as much information as possible when scheduling your appointment so that we can allow the proper amount of time for your visit. Whenever possible, please place your telephone calls yourself to ensure we provide you with the best health care possible. Should you choose to speak with a nurse regarding a medical question or issue, please allow time to consult a doctor prior to returning your call. If a nurse is unavailable, please leave a message. A doctor will return your call when available, typically later in the day. For any after hour or weekend medical concerns contact one of our physicians at (308) 746-6065.
Appointments
You may be asked to reschedule or wait until the doctor is able to work you into the schedule when you are more than 15 minutes late for your appointment. We urge you to be on time for scheduled appointments.
Emergencies
Should you happen to be in the waiting room when an emergency arises, we ask your indulgence for the delay. We hope that you will understand and find reassurance in knowing that if you were in a similar situation, immediate medical attention would be given to you. It may not always be possible to see your regular doctor at your scheduled time if an emergency arises. In the event of an emergency situation an on call doctor will be available to you or the receptionist will assist in rescheduling your appointment.
Fees
Charges for office visits are based upon the severity and complexity of your injury or illness as well as the time spent treating you. Other charges are based upon our fee schedule, which we believe is reasonable and customary. The Business Office is available during regular office hours to discuss any fee questions you have.
Payments
For your convenience our office accepts Mastercard, Visa, Discover, debit cards and personal checks. A statement of charges is sent to you regularly. A monthly finance charge of 1.33% is applied to accounts 90 days past due. Plum Creek Medical Group relies upon you to settle your account. You are ultimately responsible for all clinic fees relating to your care. Your employer must authorize all worker’s compensation appointments. They may notify us by phone or by sending a form with you to your scheduled appointment. All charges will be your responsibility until we receive the employer confirmation. We do submit to Third Party Payors (ex: auto accidents, injuries on public or private properties, etc.) when you provide all information necessary to file your claim.
Insurance
We need a copy of your current insurance card on file in order to process insurance claims efficiently. Please bring in your insurance card for every health care visit. We file claims with a variety of health care insurance companies that aid in the payment of your medical costs. Please see our Financial & Insurance page for a list of insurance companies we participate with. Should there be problems with an insurance claim we suggest that you first direct your questions to your insurance carrier. Payment for services is based on the allowable amount determined by your insurance company and will vary depending on your policy. Your policy may ask you, the subscriber, to pay a deductible, co-insurance, or co-pay amount and may have certain non-covered services. Your health insurance policy is an agreement between you and your health insurance carrier. Our Business Office will be pleased to assist you in resolving any problems or or setting up payment arrangements.
